Part Time Per Diem positions available in Raritan Day Program

Experience with behavioral and personal care required.

Various hours avalable including Monday through Friday, 9am - 5pm.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

   -  Evaluates skills and needs of each individual receiving services and makes recommendations to Program Manager

   -  Assists in planning and implementing IHP/ISP with Case Manager and Interdisciplinary Team

   -  Supervises clients in maintenance and upkeep of apartments or home

   -  Submits reports as required by Case Manager

   -  Assists in review and implementation of DHS licensing standards

   -  Assists in setting up of appointments

   -  Provides necessary transportation

   -  Ensures security and confidentiality of all assigned Alternatives, Inc. records

   -  Follows all policies and procedures relative to this position in a professional manner

   -  Ensures Alternatives, Inc's Person-Centered Philosophy by treating all clients with dignity and respect

   -  Develops and implements individualized training program for each individual with Program Manager and IDT

   -  Assists in planning and implementing Individual Behavior Plans with Clinical Specialist, Program Manager and IDT, when applicable

   -  Cooperates with Alternatives, Inc. and the N.J. Department of Human Services in all inspections or investigations

   -  Assists the Case Manager in training and supervision of staff

   -  Keeps secure apartment, staff office, Agency and vehicle keys

   -  Maintains records as assigned by Case Manager

   -  Participates in staff meetings as directed by Case Manager

   -  Availability to be contacted in emergency situations

   -  Assists in training of clients in necessary areas

   -  Performs necessary tasks for individuals

   -  Assists Case Manager in maintaining contact with outside agencies providing services to individuals

   -  Participates as a team member in planning, implementing and evaluating services provided by the Program

   -  Assists in carrying out recreation events by providing additional transportation and supervision

   -  Ensures operational effectiveness in emergency situations by providing services at Alternatives, Inc=s program sites as assigned

   -  Strong interpersonal & effective communication skills required

   -  Participates in & attends program & general staff meetings

PHYSICAL REQUIREMENT: Ability to lift up to 50 lbs occasionally. Ability to transport individuals.

High School Diplomma/GED & Experience Required

Must possess valid NJ/NY/PA driver's license

**In order to be eligible for employment, applicant must not have more than 5 points on their driving record and no DUIs within the last 3 years.**
